To request an evaluation, send the following:

• clear copies of credentials, made directly from the originals. (Documents in languages other than English should be accompanied by translations, except for the following languages: Dutch, French, German, Italian, Latin, Portuguese, Romanian and Spanish. These languages do not require translations.)  Note that in some cases we may need to request original documents or require authentication of documents.
• letter explaining the purpose of the evaluation (immigration, employment, further study, etc.) and providing date of birth (day, month, year), contact information (mailing address, telephone, fax, e-mail address, as applicable).  Include any special instructions.   (Note:  no application form is required.)  Request may be submitted by fax to 1-781-235-6831 (or 001-781-235-6831 from outside the United States) provided payment is made by credit card--see below.  Request may also be scanned and sent by e-mail (please send documents all in one file for ease of printing).   Do not, however, send credit card information by e-mail;  send it separately by fax.
• check or money order payable to Education International, Inc. for the total amount of the fee.  (If outside the United States, payment should be made by a check drawn on a United States bank or an international money order.) Or payment may be made by Visa or Master Card--please provide card number and 4-digit expiration date.  Credit card information will be secure.

Delivery of evaluation by facsimile may be requested at no extra charge, with the original to follow by mail. Delivery by courier will be made at recipient's expense.
